This programis for Indian Health Services (IHS) beneficiaries and their descendants who hold a current ProviderOne card and need transportation to medically necessary appointments.

Individuals must live within a 60-mile radius of the Cowlitz Indian Tribe Health Clinic, meet the Medicaid criteria, and have exhausted all other transportation options.
